New patients to N.C. Nephrology are scheduled by physician referrals. Once we receive the New Patient Referral Form, our office will contact the patient directly to schedule the new patient appointment.
New Patients
New patients to Wake Nephrology Associates are scheduled by physician referral. Our New Patient Referral Form is located under FORMS. Once we receive the New Patient Referral Form, our office will contact the patient directly to schedule the new patient appointment.
What to Bring to Your First Visit
- New patient materials including Patient Registration Form, Notice of Privacy Practices, Consent Signature, and Health History form. We will mail these to you in advance of your appointment, or they can be printed directly off our web site from the FORMs tab. Please complete these in advance or arrive 15 minutes early to do so.
- Your current medication bottles (including all prescriptions and over-the-counter medications).
- Insurance cards.
- Photo-ID.
- Co-pay, if applicable.
- Patients might be required to provide a urine sample for kidney related testing.

No Show Policy
If you are unable to keep your appointment, please give our office a minimum of 24 hours’ notice. If 24 hours’ notice is not given, a $25.00 no show fee will be charged for missed appointments.
Accessible to you 24 hours a day
Our providers are available 24 hours a day, seven days a week, including holidays.
Insurance
We participate in various HMO and PPO plans. We are a participating provider for Medicare and Medicaid. Patients are asked to bring insurance cards to each appointment. All patients are expected to pay co-payments at the time of service. We accept cash, checks, debit, and all major credit cards.
Hospitalized patients will receive a statement of physician fees from our office, separate from hospital billing.
If you are a self-pay patient, we have several payment plans available.
Not all services are covered benefits in all insurance contracts. Some policies require prior authorization. If your policy requires prior authorization, you should ensure that prior authorization is obtained prior to your appointment. If prior authorization is not obtained, these services will be billed to the patient. NC Nephrology reserves the right to reschedule an appointment if a prior authorization has not been obtained.
Medical Records Release
